Authority must be delegated as small-unit as possible, which means the family and local community must have separation from any central state; public health should not be dictated from D.C. or some leftist council weakling. A scared and bed-shitting leftist is going to produce public policy which assumes that all other bodies are scared bed-shitters who need the state's intervention, but the effect of that is a public whose health is degraded to that of the "immunocompromised" and obese leftist shut-in.
