So what changed? Antibiotics and vaccinations. But since no one even catches the old plagues anymore, we can rule out antibiotics, which is reactive.

Thus, the thing mostly responsible for the world not being a germ hellhole any more, by unavoidable logic, is vaccination.

For instance, chickenpox isn't vaccinated against like MMR are, and is still a fairly common part of childhood.

On the flipside, smallpox has also been eradicated from places where the hygiene levels are crappy at best (India is not a clean country, for example)
