Brain trauma is for example really hard for the brain to understand. It simply does not know it is hurt. Similar to dogs that often have no awareness of going blind. Everything is the same except bumping into things happens more often for no descernable reason. People with neglect have similar issues. The other half of the plate is simply not there and has never been there.
